Treatment For Advanced Stage Head And Neck Melanoma
For patients with clinical evidence of metastases of their melanoma or with recurrent disease, staging with imaging studies will be performed. If the metastasis is relatively contained, the patient will be evaluated for surgical resection. This may involve removal of lymph nodes and one of the major salivary glands. Our surgeons work closely with the head and neck neuroradiologists for surgical planning and with the medical oncologists for subsequent adjuvant treatment planning.
Treating Stage Iii Melanoma
These cancers have already reached the lymph nodes when the melanoma is first diagnosed. Surgical treatment for stage III melanoma usually requires wide excision of the primary tumor as in earlier stages, along with lymph node dissection.
After surgery, adjuvant treatment with an immune checkpoint inhibitor or with targeted therapy drugs may help lower the risk of the melanoma coming back. Other drugs or perhaps vaccines may also be recommended as part of a clinical trial to try to reduce the chance the melanoma will come back. Another option is to give radiation therapy to the areas where the lymph nodes were removed, especially if many of the nodes contain cancer.
If melanoma tumors are found in nearby lymph vessels in or just under the skin , they should all be removed, if possible. Other options include injections of the T-VEC vaccine , Bacille Calmette-Guerin vaccine, or interleukin-2 directly into the melanoma radiation therapy or applying imiquimod cream. For melanomas on an arm or leg, another option might be isolated limb perfusion or isolated limb infusion . Other possible treatments might include targeted therapy , immunotherapy, or chemotherapy.
Some people with stage III melanoma might not be cured with current treatments, so they may want to think about taking part in a clinical trial of newer treatments.
Treating Stage Iv Melanoma
Stage IV melanomas have already spread to distant lymph nodes or other areas of the body. Skin tumors or enlarged lymph nodes causing symptoms can often be removed by surgery or treated with radiation therapy.
Metastases in internal organs are sometimes removed, depending on how many there are, where they are, and how likely they are to cause symptoms. Metastases that cause symptoms but cannot be removed may be treated with radiation, immunotherapy, targeted therapy, or chemotherapy.
The treatment of widespread melanomas has changed in recent years as newer forms of immunotherapy and targeted drugs have been shown to be more effective than chemotherapy.
Immunotherapy drugs called checkpoint inhibitors such as pembrolizumab or nivolumab are typically the first drugs tried, especially in people whose cancer cells do not have BRAF gene changes. These drugs can shrink tumors for long periods of time in some people. Ipilimumab , a different type of checkpoint inhibitor, is not typically used by itself as the first treatment, although it might be combined with nivolumab or pembrolizumab. This slightly increase the chances that the tumor will shrink, although itâs also more likely to result in serious side effects, which needs to be considered carefully. People who get any of these drugs need to be watched closely for serious side effects..
Itâs important to carefully consider the possible benefits and side effects of any recommended treatment before starting it.

How To Look For Melanoma On Your Scalp
Melanoma on the scalp is one of the more dangerous types of skin cancer, as it forms in an area thats not easily detectable and can grow at a much faster rate due to an abundant amount of blood vessels and tissues. A regular skin self-exam should always include inspecting the scalp, as this part of the body is constantly exposed to the sun and can burn easily, especially if you are bald or have thin hair.
Can You Use Regular Sunscreen On Your Scalp
That brings us to our next subject: what type of SPF should you use on your scalp? Traditional sunscreen lotions are known to be somewhat oily and can leave a greasy residue.
If you already have thinning hair, the last thing you want to do is add more oil to your scalp to make it look even thinner.
Thankfully, there are other options out there that provide the same level of SPF without the greasiness.
Powder sunscreen is an option for the scalp because it blends in naturally with your hair and can even have a mattifying effect. Think dry shampoo, but for your scalp health.
Sunscreen spray or sunscreen mist is another option that is specifically created to protect your hair and scalp from UVR, and is probably your best bet.
Theres also stick sunscreen, a third option because of its convenient packaging and easy application. However, we should note its really only useful on hairlines or parts, and isnt really practical for applying to the scalp skin under your hair.
Its also important to note that the two most common types of sunscreen chemical and physical and each protect you differently.
Chemical sunscreens mechanism of action involves absorbing the suns rays. They usually contain one or more of the following: avobenzone, octisalate, oxybenzone, octocrylene, octinoxate or homosalate. Chemical sunscreens tend to be easier to rub into your skin without the white residue often found in more typical sunscreens.

What Is Cutaneous Melanoma
Melanoma is the most dangerous form of skin cancer. It arises when pigment-forming cells, called melanocytes, in the basal layer of the skin sustain genetic alterations that cause these cells to multiply in an uncontrolled manner. The most common cause of these genetic alterations is extensive exposure to the ultraviolet radiation from the sun or from tanning booths.
In addition to multiplying in an uncontrolled manner, these altered melanocytes can invade deep into the underlying tissue and spread to other parts of the body including sites like lymph nodes, the lungs, the liver, bone, and the brain. When melanoma is detected early and treated, it is very curable. However, if it has metastasized to lymph nodes or to distant sites , it can be fatal.
It Was Official: I Had Melanoma On My Scalp
I remember never feeling nervous about what my biopsy would reveal. Looking back though, I should have been panicked. For someone who felt so confident about life, one cannot imagine how surprised I was to learn two weeks later that I had stage 2 advanced nodular melanoma on my scalp.
Although I had a million questions at the time, I still wasn’t concerned. In fact, I remember telling my mom, “No need to cry. It was just a tiny spot on my head. Don’t worry so much.” The next day, everything changed.

Can You Die From Skin Cancer On Your Head
Yes. You can die from untreated skin cancer on your head.
However, do not panic yet. Most skin cancer on the head or skin cancer on the scalp is highly treatable, especially during the early stages.
If you are still in the earliest stages of treatment, such as for Stage I melanoma, there is a low risk of metastasis or recurrence.
According to Healthline and other sources, the five-year survival rate for the earliest stages of melanoma on the scalp is as high as 97%.

Types Of Skin Malignancies:
- Melanoma the least common form of skin cancer, but responsible for more deaths per year than squamous cell and basal cell skin cancers combined. Melanoma is also more likely to spread and may be harder to control.
- Nonmelanoma malignancies:
- Squamous cell cancer the second-most common skin cancer. It’s more aggressive and may require extensive surgery, depending on location and nerve involvement.
- Basal cell cancer the most common form of skin cancer. It is rarely fatal but can be locally aggressive.
These skin malignancies are typically caused by ultraviolet radiation from exposure to the sun and tanning beds.
How Common Is Scalp Melanoma

- Exposure may just be at the crown or vertex where your hair whorl is.
- Part lines that run from hair whorl to forehead are also at risk for people who have natural or defined parts.
- If you have significant hair thinning then UV exposure may be more extensive.
Most people have experienced sunburn on their scalp at least once. As hair thins with age, it starts at the top. The sun’s exposure above is relentless. Intermittent sun exposure versus long term/chronic sun exposure generally influences the risk of getting melanoma. We also know the more moles a person has, the greater their melanoma risk.

When Should You Call Your Doctor
if you have any of these symptoms:
- You have trouble breathing or swallowing.
- You cough up or spit up blood.
- You have blood in your vomit or stool .
- Your urine or stool is black, and the blackness isn’t caused by taking iron or Pepto-Bismol.
The most important warning sign for melanoma is a change in size, shape, or color of a mole or other skin growth . Call your doctor if you have:
- Any change in a mole, including size, shape, color, soreness, or pain.
- A bleeding mole.
- A discolored area under a fingernail or toenail not caused by an injury.
- A general darkening of the skin unrelated to sun exposure.
If you have been diagnosed with cancer, be sure to follow your doctor’s instructions about calling when you have problems, new symptoms, or symptoms that get worse.
When Might Radiation Therapy Be Used
Radiation therapy is not needed for most people with melanoma on the skin, although it might be useful in certain situations:
- It might be an option to treat very early stage melanomas, if surgery can’t be done for some reason.
- Radiation can also be used after surgery for an uncommon type of melanoma known as desmoplastic melanoma.
- Sometimes, radiation is given after surgery in the area where lymph nodes were removed, especially if many of the nodes contained cancer cells. This is to try to lower the chance that the cancer will come back.
- Radiation can be used to treat melanoma that has come back after surgery, either in the skin or lymph nodes, or to help treat distant spread of the disease.
- Radiation therapy is often used to relieve symptoms caused by the spread of the melanoma, especially to the brain or bones. Treatment with the goal of relieving symptoms is called palliative therapy. Palliative radiation therapy is not expected to cure the cancer, but it might help shrink it or slow its growth for a time to help control some of the symptoms.

Treatment of stage I melanoma may include the following:
- Surgery to remove the tumor and some of the normaltissue around it. Sometimes lymph node mapping and removal of lymph nodes is also done.
- A clinical trial of new ways to find cancercells in the lymph nodes.

Melanoma Is A Disease In Which Malignant Cells Form In Melanocytes
The skin is the bodys largest organ. It protects against heat, sunlight, injury, and infection. Skin also helps control body temperature and stores water, fat, and vitamin D. The skin has several layers, but the two main layers are the epidermis and the dermis . Skin cancer begins in the epidermis, which is made up of three kinds of cells:
- Squamous cells: Thin, flat cells that form the top layer of the epidermis.
- Basal cells: Round cells under the squamous cells.
- Melanocytes: Cells that make melanin and are found in the lower part of the epidermis. Melanin is the pigment that gives skin its natural color. When skin is exposed to the sun or artificial light, melanocytes make more pigment and cause the skin to darken.
The number of new cases of melanoma has been increasing over the last 30 years. Melanoma is most common in adults, but it is sometimes found in children and adolescents.
Can Melanoma Be Cured
Melanoma that’s caught early, when it’s still on the surface of the skin, can be cured.
Untreated melanoma can grow downward into the skin until it reaches the blood vessels and lymphatic system. This lets it travel to distant organs, like the lungs or the brain. That’s why early detection is so important.

How Is Melanoma Diagnosed
Your doctor will check your skin to look for melanoma. If your doctor thinks that you have melanoma, he or she will remove a sample of tissue from the area around the melanoma. Another doctor, called a pathologist, will look at the tissue to check for cancer cells.
If your biopsy shows melanoma, you may need to have more tests to find out if it has spread to your lymph nodes.
Can Changing My Diet Help Prevent Melanoma
The American Cancer Society advocates eating a plant-based diet over an animal-based diet as part of a healthy plan to avoid all cancers. Growing evidence suggests that plants pack a powerful punch in any fight against cancer because they’re nutritious, cholesterol-free and fiber-rich.
Theres no doubt that a healthy diet can protect your immune system. Having a strong immune system is important to help your body fight disease. Some research has shown that a Mediterranean diet is a healthy choice that may help prevent the development of cancer. Talk to your healthcare provider about the role food plays in lowering your cancer risks.
Some skin and immune-system healthy foods to consider include:
- Daily tea drinking: The polyphenols in tea help strengthen your immune system. Green tea contains more polyphenols than black tea.
- High vegetable consumption: Eating carrots, cruciferous and leafy vegetables is linked to the prevention of cutaneous melanoma.
- Weekly fish intake: Study participants who ate fish weekly seemed to avoid developing the disease when compared to those who did not eat fish weekly.

What Is Radiation Therapy
After surgery your doctors may recommend radiation therapy to help improve your outcome.
Radiation therapy uses x-rays to kill cancer cells by damaging their DNA. Normal cells can repair damage to their DNA, but cancer cells are less able to do this and therefore die. The dead cancer cells are then broken down and eliminated by the bodys natural processes.
Since radiation therapy damages normal cells as well as cancer cells, treatment must be carefully planned to allow the normal cells to repair themselves and minimise side effects.
The total dose of radiation and the number of treatments you need will depend on the size and location of your melanoma, your general health and other medical treatments youre receiving.
The radiation used for cancer treatment can come from a machine outside your body or it might come from radioactive material placed in your body near the cancer cells.
