What Does Nail Melanoma Look Like Skin Cancer Can Hide As Line On Nail
Karolina Jasko has a family history of melanoma, so she’s no stranger to paying attention to her skin. Her mother who battled the deadliest form of skin cancer twice and recovered has always been vigilant about checking Jaskos moles for any changes.
But melanoma was still able to sneak up on Jasko in a spot neither she nor her mom suspected: one of Jaskos nails.
Experts like Dr. Vishal Patel, assistant professor of dermatology at the George Washington University School of Medicine & Health Sciences in Washington, D.C., worry such cases may be on the rise with the popularity of gel manicures that require the polish to be hardened under ultraviolet light.
Its like tanning beds for your hands, Patel, who is also the director of the cutaneous oncology program at the GW Cancer Center, told TODAY. He was not involved in Jaskos case, but commented in general.
Were seeing a lot of patients having not only melanomas, but all types of skin cancers around the finger tips and the cuticles.
A Nail Streak Appears
In high school, Sylvia played a lot of sports so when the tan streak appeared on her right thumbnail she thought maybe it was a blood blister or a bruise. When she visited a podiatrist for an ingrown toenail, the doctor recommended that Sylvia visit a dermatologist to have it examined.
basically said its most likely a mole in your nail bed and its probably not going to be of concern because the alternative was like a 1% chance it was this cancer, she said. They basically told me to wait and watch and see if it got darker, grew wider.
For the next several years, Sylvia thought little of the streak.
I wasnt concerned. I was like, What are the odds? Sylvia recalled. I would say, Oh I was just told it was a mole under my nail bed. I grew to accept that it was just part of my hand.
Last fall, a former co-worker, reached out to Sylvia after reading something about subungual melanoma and encouraged Sylvia to see a dermatologist.
She saw a picture that looked like my thumb and so she sent it to me, You need to get this looked at, Sylvia said. Its almost like you dont want to face it at that point because its scary.
Sylvia started researching it and in January she made an appointment with a dermatologist.
I owe it to my co-worker for creating a voice in the back of my head to get it looked at, Sylvia said.
But the still dermatologist wasnt sure about conducting a biopsy.

What Is Melanoma Of The Nail Unit
Melanoma of the nail unit is usually a variant of acral lentiginous melanoma, a malignant melanoma arising from the palms of the hands and soles of the feet. Other rare forms of melanoma that may arise in the nail unit are nodular melanoma or desmoplastic melanoma.
Melanoma of the nail unit most often affects the great toe and thumbnail, accounting for 7590% of cases. However, any nail on the finger or toe may be involved.
The term includes:
- Subungual melanoma
- Ungual melanoma
- Periungual melanoma .

What Causes Melanoma In The Eye
The exact cause of ocular melanoma is not known, but its thought that a combination of genetic and environmental factors may play a role.
Research is inconclusive about whether ultraviolet light exposure increases the risk of ocular melanoma like it does with skin melanoma.
About 50 percent of people with ocular melanoma have a loss of genetic information on chromosome 3.
People with a strong family history of eye cancers may have a rare genetic mutation in their BAP1 gene. The BAP1 gene is classified as a tumor suppressor gene and slows down cell division.
The genes EIF1AX and SRSF2/SF3B1 also seem to occur frequently in people with ocular melanoma.
Although its not clear why ocular melanoma develops, certain people are at a higher risk. Risk factors include:

Subungual Melanoma Causes Diagnosis And Treatment
Subungual melanoma, also referred to as cancer of the nail unit, is a type of malignancy that arises in the tissues of the nail bed. Melanoma is a type of cancer that develops in cells called melanocytes. Melanocytes are cells that produce melanin, the pigment that gives skin, hair, and eyes their color.
While subungual melanomas are most commonly found on the thumb or big toe, they can occur on any of the fingernails or toenails. They’re often misdiagnosed as a fungal infection due to their characteristic changes in color and nail texture.
Subungual melanoma is a relatively uncommon conditionaffecting 0.7% to 3.5% of people with malignant melanomas worldwideand tends to occur more in darker-skinned individuals, including African-American, Asian, and Hispanic populations. It’s also more common in older people, in a woman’s 60s and a man’s 70s.
Treatment For Melanoma Under Nails

Overview And Scope Of The Guidelines
Melanoma is a life threatening but potentially treatable form of cancer if diagnosed and managed at an early stage. Guidelines have been published to assist healthcare workers in the recognition of malignant melanoma of the skin . However, early melanoma arising on the foot, particularly within the nail unit and on the plantar surface, can be difficult to recognise. Consequently, this can lead to delays in diagnosis. Melanoma arising on the foot carries a particularly poor prognosis when compared to melanoma arising at other body sites . As there are no consistent features of an early melanoma, these guidelines have been drafted to alert health care workers to the signs which may suggest melanoma and therefore warrant a specialist referral. A melanoma recognised and diagnosed at an early stage can dramatically increase a patients chances of survival.
Differences In Appearance Between Blood Under A Toenail And A Melanoma: Under More Scrutiny
Association with trauma. The trauma that leads to blood under a toenail isnt always blunt and painful at the time of occurrence.
It could be as seemingly innocuous as jogging downhill in footwear that isnt roomy enough. The patient might not even recall the trauma.
Though subungual hematoma often affects both big toes at the same time, it CAN occur in only one toe.
If your right foot is a little bigger than the left, the shoe on your right foot will have a tighter fit that may result in a hematoma, while the shoe on the left wont.
More than one toenail. If the appearance of the dark pigment in a big toe coincides with a similar-looking pigment in another toe either on that foot or the other then this points to a benign cause.
Blood grows out. If you view a hematoma every several days, youll notice that the space between the portion closest to the cuticle and the cuticle is getting bigger, or showing more nail.
As the nail is pushed out, the blood underneath goes with it. So if you noticed the pigment when it was first visibly connected to the cuticle area, in a few weeks it will be closer to the center of the nail, then after several more weeks itll be near the top of the nail. It can take several months to finally grow out.
Eventually the nail will be long enough such that when you trim it, youll also be trimming out the old dried blood.
A melanoma will stay firmly adhered to its place it will not move upward as the nail grows.

Prognosis And Survival Rate
- ALM can look a lot like lentigo maligna melanoma but has been noted to be more aggressive and have a worse prognosis.
- Prognosis is much better the sooner it is diagnosed.
- Prognosis is directly related to the size and depth of the tumor or lesion.

It is important to note that while longitudinal melanonychia only occurs in around 1% of white-skinned individuals, it is very common in dark-skinned races, occurring in an estimated 100% of African-Americans by the age of 50 and around 20% of Asians. This may lead to misdiagnosis or delayed diagnosis of nail unit melanoma, and therefore poorer outcomes.

How Fast Does Subungual Melanoma Grow
Subungual melanoma often begins as a visible pigment band on the length of the nail plate . For weeks or months, the pigment band:
- Grows, especially in the proximal termination .
- Becomes more irregular in pigmentation, including light brown and dark brown.
- Extends by becoming involved in the adjacent nail fold .
- Can develop a lump, ulcerate or bleed.
- Can lead to thinning, cracks or deformation of the nail plate .
However, up to half of all cases of subungual melanoma are amelanotic . Nail melanoma may form a lump under the surface of the nail, lifting it . Sometimes it may look like a verrucous wart. It is usually painless, but the advanced tumor invades the underlying bone, which can origin a grim pain.
How Does Skin Cancer Appear On Nails
Skin cancer under the nail generally falls into one of three categories:
- Acral lentiginous melanoma, the most common form of skin cancer on nails, looks like a dark band that runs from the nail matrix outward toward the edge of your nail.
- Nodular melanoma takes the form of a bump that may originate beneath the nail. Onycholysis where your nail is pushed up and detached is a common symptom.
- Demoplastic melanoma, while most commonly found near the neck and head, can also be found on skin near the nails. This type of melanoma is named for the fibrous tissue that surrounds the tumor. It most often presents as a skin-colored or pink bump with a smooth or irregular surface, but it may take on darker shades.
