Infiltrative Basal Cell Carcinoma

Infiltrative basal cell carcinoma occurs when a tumor makes its way into the dermis via thin strands between collagen fibers. This aggressive type of skin cancer is harder to diagnose and treat because of its location. Typically, infiltrative basal cell carcinoma appears as scar tissue or thickening of the skin and requires a biopsy to properly diagnose.
To remove this type of basal cell carcinoma, a specific form of surgery, called Mohs, is used. During a Mohs surgery, also called Mohs micrographic surgery, thin layers of skin are removed until there is no cancer tissue left.

Superficial basal cell carcinoma, also known as in situ basal-cell carcinoma, tends to occur on the shoulders or the upper part of the torso, but it can also be found on the legs and arms. This type of cancer isnt generally invasive because it has a slow rate of growth and is fairly easy to spot and diagnose. It appears reddish or pinkish in color and may crust over or ooze. Superficial basal cell carcinoma accounts for roughly 15%-26% of all basal cell carcinoma cases.
Can Squamous Cell Carcinoma Be Prevented
The best way to prevent SCC is to avoid sunburn. Avoid going outin the sun when the UV Index is higher than 3, such as in the middle of theday. Seek shade, wear a hat, sunglasses and clothing that protects you from thesun, and always use an SPF30+ sunscreen. Do not go to tanningsalons.

What Is Skin Cancer
Skin cancer refers to the uncontrolled growth of cells in the skin. Skin cancer is the commonest type of cancer in the United States. Your skin acts as a protective barrier containing several types of cells. Depending on the cell from which skin cancer originates, it can be of several types. The most common types of skin cancers are basal cell carcinoma and squamous cell carcinoma. These two types of skin cancers are highly curable unlike the third most common skin cancer called melanoma. Melanoma is the most dangerous skin cancer that causes many deaths. Even curable skin cancers can cause significant disfigurement. Other types of skin cancers include lymphoma of the skin, Kaposi sarcoma, and Merkel cell skin cancer. Knowing the type of skin cancer is crucial for the doctor to decide treatment.

How Is Skin Cancer Treated
Treatment of skin cancer depends on the type and extent of the disease. Treatment is individualized and is determined by the type of skin cancer, its size and location, and the patient’s preference.
Standard treatments for non-melanoma skin cancer include:
- Mohs surgery : Skin-sparing excision of cancer with complete peripheral and deep margin assessment.
- Excision.
- Electrodesiccation and curettage: Scraping away the skin cancer cells followed by electrosurgery.
- Cryosurgery.
- Drugs .
Standard treatments for melanoma include:
- Wide surgical excision.
- Sentinel lymph node mapping : to determine if the melanoma has spread to local lymph nodes.

Actinic Keratosis Signs And Symptoms
Many people have actinic keratosis , also called solar keratosis, on their skin. It shows that youâve had enough sun to develop skin cancer, and it is considered a precursor of cancer, or a precancerous condition.
Usually AK shows up on the parts of your body that have received the most lifetime sun exposure, like the face, ears, scalp, neck, backs of the hands, forearms, shoulders and lips.
Some of the same treatments used for nonmelanoma skin cancers are used for AK to ensure it does not develop into a cancerous lesion.
Appearance
This abnormality develops slowly. The lesions are usually small, about an eighth of an inch to a quarter of an inch in size. You may see a few at a time. They can disappear and later return.
- AK is a scaly or crusty bump on the skinâs surface and is usually dry and rough. It can be flat. An actinic keratosis is often noticed more by touch than sight.
- It may be the same color as your skin, or it may be light, dark, tan, pink, red or a combination of colors.
- It can itch or produce a prickling or tender sensation.
- These skin abnormalities can become inflamed and be encircled with redness. Rarely, they bleed.

How Is Skin Cancer Of The Head And Neck Diagnosed
Diagnosis is made by clinical exam and a biopsy. Basal cell and squamous cell cancers are staged by size and extent of growth. Basal cell cancers rarely metastasize to lymph nodes, but they can grow quite large and invade local structures. Squamous cell cancers have a much higher incidence of lymph node involvement in the neck and parotid gland and can spread along nerves.
Melanoma is staged, based not on size but on how deeply it invades the skin layers. Therefore, a superficial or shave biopsy will not provide accurate staging information used to guide treatment. Melanomas can have a very unpredictable course and may spread to distant organs. Melanomas with intermediate thickness often require sentinel node biopsy, a surgical procedure performed by a head and neck surgeon, to determine if microscopic spreading to lymph nodes has occurred.

Basal Cell Carcinoma Pictures
Basal cell carcinoma usually appears in areas of the skin previously exposed to high levels of UV radiation such as the head, neck, ears and the back of the arms and hands. It is common in exposed skin of outdoor workers or people who have used sun tanning beds in the past.
As the basal cell carcinoma pictures below indicate, this type of skin cancer usually shows as a fleshy coloured bump that does not disappear over time and tends to grow slowly in size, eventually breaking down and ulcerating.

Curettage Electrodesiccation And Cryotherapy
Some dermatologists perform curettage, electrodesiccation, and cryotherapy to treat skin cancer. These are considered to be destructive techniques that are best suited for small, superficial carcinomas with definite borders. During the procedure, layers of skin cells are scraped away using a curette. Any remaining cancer cells are destroyed with the use of an electric needle.
In some cases, liquid nitrogen or cryotherapy is used to freeze the margins of the treatment area. Extremely low temperatures kill the malignant skin cells and create a wound, which will heal in a few weeks. The treatment may leave scars that are flat and round, similar to the size of the skin cancer lesion.

Sun exposure causes most of the skin changes that we think of as a normal part of aging. Over time, the sun’s ultraviolet light damages the fibers in the skin called elastin. When these fibers break down, the skin begins to sag, stretch, and lose its ability to go back into place after stretching. The skin also bruises and tears more easily in addition to taking longer to heal. So while sun damage to the skin may not be apparent when you’re young, it will definitely show later in life. Will I Need Radiotherapy Or Chemotherapy
It is unusual to need radiotherapy for a simple BCC radiotherapy is generally reserved as an additional treatment for very extensive skin cancers. You will not need chemotherapy. Rarely BCCs are treated with radiotherapy only there are downsides to this and it is not the usual approach to treatment.

Squamous cell carcinoma in situ, also known as Bowens disease, is a precancerous condition that appears as a red or brownish patch or plaque on the skin that grows slowly over time. The patches are often found on the legs and lower parts of the body, as well as the head and neck. In rare cases, it has been found on the hands and feet, in the genital area, and in the area around the anus.
Bowens disease is uncommon: only 15 out of every 100,000 people will develop this condition every year. The condition typically affects the Caucasian population, but women are more likely to develop Bowens disease than men. The majority of cases are in adults over 60. As with other skin cancers, Bowens disease can develop after long-term exposure to the sun. It can also develop following radiotherapy treatment. Other causes include immune suppression, skin injury, inflammatory skin conditions, and a human papillomavirus infection.
Bowens disease is generally treatable and doesnt develop into squamous cell carcinoma. Up to 16% of cases develop into cancer.
Who Gets Skin Cancer And Why
Sun exposure is the biggest cause of skin cancer. But it doesn’t explain skin cancers that develop on skin not ordinarily exposed to sunlight. Exposure to environmental hazards, radiation treatment, and even heredity may play a role. Although anyone can get skin cancer, the risk is greatest for people who have:
- Fair skin or light-colored eyes
- An abundance of large and irregularly-shaped moles
- A family history of skin cancer
- A history of excessive sun exposure or blistering sunburns
- Lived at high altitudes or with year-round sunshine
- Received radiation treatments
