Healing By Secondary Intention

Secondary intention healing involves allowing the wound to heal spontaneously without suturing the edges together. During the healing time the wound needs to be cleaned regularly and ointment is typically applied several times per day to keep the wound moist. Depending on the size of the wound, healing can take weeks to months to complete, but eventually the open wound is covered with a new layer of skin that has grown in from the edges. Wounds that heal by secondary intention typically have more scar contraction and may have a lighter color or slight depression in height compared to surrounding skin. For most locations of the head and neck, secondary intention is not an ideal method of wound closure for these reasons. In specific circumstances, however, secondary intention healing may be recommended as a simple method of wound closure that does not require sutures or additional incisions.

Research And Clinical Trials

Clinical trials are currently being conducted using biological therapy, also called biological response modifier therapy, or immunotherapy. Biological therapy tries to get your own body to fight cancer by using materials made by your own body, or made in a laboratory, to boost, direct, or restore your body’s natural defenses against disease.

Research efforts in this field at Northwestern are also burgeoning. Northwestern is one of 11 collaborating sites studying hedgehog inhibitors and their role in treating basal cell cancers, which are currently treated by surgery.

It is the hope that hedgehog-inhibiting drugs could prevent, cure, or provide alternative treatment for those who have cancer that is inoperable or cannot be treated with radiation .

There Are Three Ways That Cancer Spreads In The Body

Cancer can spread through tissue, the lymph system, and the blood:

  • Tissue. The cancer spreads from where it began by growing into nearby areas.
  • Lymph system. The cancer spreads from where it began by getting into the lymph system. The cancer travels through the lymph vessels to other parts of the body.
  • Blood. The cancer spreads from where it began by getting into the blood. The cancer travels through the blood vessels to other parts of the body.

Certain Factors Affect Prognosis And Treatment Options

The prognosis for squamous cell carcinoma of the skin depends mostly on the following:

Treatment options for basal cell carcinoma and squamous cell carcinoma of the skin depend on the following:

  • The type of cancer.
  • The stage of the cancer, for squamous cell carcinoma.
  • The size of the tumor and what part of the body it affects.
  • The patients general health.

Tests Or Procedures That Examine The Skin Are Used To Diagnose Basal Cell Carcinoma And Squamous Cell Carcinoma Of The Skin

10 Ways To Remove Facial Skin Lesions

The following procedures may be used:

  • Physical exam and health history: An exam of the body to check general signs of health, including checking for signs of disease, such as lumps or anything else that seems unusual. A history of the patients health habits and past illnesses and treatments will also be taken.
  • Skin exam: An exam of the skin for bumps or spots that look abnormal in color, size, shape, or texture.
  • Skin biopsy: All or part of the abnormal-looking growth is cut from the skin and viewed under a microscope by a pathologist to check for signs of cancer. There are four main types of skin biopsies:
  • Shave biopsy: A sterile razor blade is used to shave-off the abnormal-looking growth.
  • Punch biopsy: A special instrument called a punch or a trephine is used to remove a circle of tissue from the abnormal-looking growth. Enlarge Punch biopsy. A hollow, circular scalpel is used to cut into a lesion on the skin. The instrument is turned clockwise and counterclockwise to cut down about 4 millimeters to the layer of fatty tissue below the dermis. A small sample of tissue is removed to be checked under a microscope. Skin thickness is different on different parts of the body.
  • Incisional biopsy: A scalpel is used to remove part of a growth.
  • Excisional biopsy: A scalpel is used to remove the entire growth.

Knowing What To Avoid

  • 1Avoid using mole removal creams. These creams are often sold online, marketed as a cheap, noninvasive alternative to surgical removal. In fact, mole removal creams can end up leaving deep pockets in your skin, since they go beyond the mole and dig into the skin underneath, causing irreparable damage. The small scar left behind by surgical removal is minimal in comparison.XTrustworthy SourcePubMed CentralJournal archive from the U.S. National Institutes of HealthGo to source
  • In addition, mole removal creams donât address the issue of whether or not the mole is cancerous. Applying it to a cancerous mole can be very dangerous; cancerous cells could remain and end up growing out of control without your knowledge.
  • Donât use any type of cream or other product without consulting a doctor first.
  • 2Donât ignore a mole thatâs changing. If you donât like the prospect of having surgery, you might be tempted to let your mole be and forget about it. Thatâs usually fine, unless you notice that the mole has changed over time. A changing mole can be a sign of the presence of cancer cells, so you should have any mole checked out by your doctor. Use the ABCDE guide to examine your mole. • How Is Basal Cell Skin Cancer Treated When It Grows Deep Or Spreads

    While this skin cancer tends to grow slowly, early treatment is recommended. Without treatment, BCC can grow deep, destroying what lies in its way. This can be disfiguring. The medical term for this is advanced basal cell carcinoma.

    Its also possible for BCC to spread to other parts of your body, but this is rare. When the cancer spreads, it typically travels first to the lymph nodes closest to the tumor. From there, it tends to spread through the blood to bones, the lungs, and other parts of the skin. When this skin cancer spreads, it is called metastatic basal cell carcinoma.

    For cancer that has grown deep or spread to the closest lymph nodes, treatment may involve:

    • Surgery to remove the tumor

    • Follow-up treatment with radiation to kill any remaining cancer cells

    For some patients, medication that works throughout the body may be an option. Medication may also be used to treat cancer that:

    • Returns after surgery or radiation treatments

    • Has spread to another part of the body

    Two such medications have been approved by the U.S. Food and Drug Administration . Both come in pill form and are taken every day. A patient only stops taking the medication if the cancer starts to grow, or the side effects become too severe.

    The two medications are:

    • Sonidegib

    • Vismodegib

    In clinical trials, these medications have been shown to stop or slow down the spread of the cancer and shrink the cancerous tumors in some patients.

    Skin Cancer On The Face: Types And Prevention

    Casey Gallagher, MD, is board-certified in dermatology. He is a clinical professor at the University of Colorado in Denver, and co-founder and practicing dermatologist at the Boulder Valley Center for Dermatology in Colorado.

    Because it is exposed to the sun more than other parts of the body, the skin on your face is especially vulnerable to skin cancer. And skin cancer on the face can be mistaken for other conditionssuch as age spots, pimples, scarring, acne, styes, and cysts.

    Skin cancers that tend to occur more often on the face include actinic keratosis, basal cell carcinoma, and squamous cell carcinoma. The face is also a common site of melanoma and there are several other lesser-common skin cancers that can affect the face. The risk of getting skin cancers on the face increases with high amounts of sun exposure and other ultraviolet light exposure.;

    About 75% of non-melanoma skin cancers occur on the head or neck.

    Skin cancer occurs when cells in the skin’s layers become damaged in ways that cause them to look and act differently than the normal healthy cells around them and start to grow out of control. UV rays play a major role in damaging cells by causing gene mutations.;

    You can watch for signs of skin cancer on your face by paying attention to new or odd-looking spots or feeling growths, splotches, or moles.

    What Is The Likely Outcome For Someone Who Has Bcc

    When found early and treated, this skin cancer can often be removed. However, this skin cancer can return. You also have a higher risk of developing another BCC or other type of skin cancer.

    Thats why self-care becomes so important after treatment for BCC. Youll find the self-care that dermatologists recommend at, Basal cell carcinoma: Self-care.

    Staging For Basal Cell Carcinoma And Squamous Cell Carcinoma Of The Skin Depends On Where The Cancer Formed

    Staging for basal cell carcinoma and squamous cell carcinoma of the eyelid is different from staging for basal cell carcinoma and squamous cell carcinoma found on other areas of the head or neck. There is no staging system for basal cell carcinoma or squamous cell carcinoma that is not found on the head or neck.

    Surgery to remove the primary tumor and abnormal lymph nodes is done so that tissue samples can be studied under a microscope. This is called pathologic staging and the findings are used for staging as described below. If staging is done before surgery to remove the tumor, it is called clinical staging. The clinical stage may be different from the pathologic stage.

    What Should I Expect Before During And After Excision

    The best type of doctor and setting for excision will depend on tumor size, location, and type. Thin melanomas and low-risk non-melanoma skin cancers can be excised in a doctors office. A numbing medicine will be injected at the tumor. After the procedure, you will be given instructions on how to care for the incision.

    Excision of thicker melanomas may be done at the same time as sentinel lymph node biopsy. This procedure is usually done at the hospital under general anesthesia. Often it is an outpatient surgery, although sometimes patients stay overnight.

    Oral Medications For Advanced Bcc

    It is rare for skin cancer to reach advanced stages, but when it does, oral medications may help. In addition to chemotherapy, targeted drugs may be used to treat advanced skin cancer. Targeted therapy means that the medication is able to directly target the cancer cells without destroying healthy cells. This can help to reduce side effects from treatment.;

    Vismodegib and sonidegib are hedgehog pathway inhibitors that work to prevent cancer cells from growing and spreading. The capsules are taken once per day and may be considered after surgery and other treatments. These medications come with several possible side effects and should never be taken during pregnancy since they can affect fetal growth.

    Cetuximab is an EGFR inhibitor that can help to stop the spread of cancerous squamous cells. Its possible side effects include skin infections, diarrhea, mouth sores, and loss of appetite.;

    Basal Cell Carcinoma

    Article by Karen Choi, MD

    Basal cell carcinoma is the most common skin cancer and usually develops in sun-exposed areas of the skin. Men, older people , and those with a history of increased sun exposure are at higher risk for developing this cancer.

    A classic example of an early basal cell carcinoma. The lesion is flesh colored with a round shape and smooth borders

    Basal cell carcinoma generally does NOT spread to lymph nodes or other parts of the body. There are some genetic syndromes that cause the appearance of multiple BCCs such as Gorlin syndrome, also known as Nevoid basal cell carcinoma syndrome .

    There are three main types of BCC:

    Basal cell carcinoma of the ear. The larger lesion is flesh colored with a pearly appearance and there are smaller lesions deeper within the ear as a result of the cancer spreading along the ear cartilage.

    Mohs Surgery Scar Minimized

    Mohs surgery differs from other techniques since the microscopic examination of all excised tissues during the surgery eliminates estimation about how far out or deep the roots of the skin cancer go. This process ensures two things: that the doctor got all of the cancer and that he took out only the amount that was necessary. The goal is to minimize the Mohs surgery scar, but it is impossible to know exactly how your skin will heal. By comparison, non-Mohs skin cancer excision usually ranges from one to two inches and may require multiple surgeries if all of the skin cancer was not removed.

    Surgical Procedures For Basal & Squamous Cell Skin Cancers

    Basal or squamous cell skin cancers may need to be removed with procedures such as electrodessication and curettage, surgical excision, or Mohs surgery, with possible reconstruction of the skin and surrounding tissue.

    Squamous cell cancer can be aggressive, and our surgeons may need to remove more tissue. They may also recommend additional treatments for advanced squamous cell cancer, such as medications or radiation therapyenergy beams that penetrate the skin, killing cancer cells in the body.

    Basal cell cancer is less likely to become aggressive, but if it does, our doctors may use surgery and other therapies to treat it.
