ISTANBULGabapentin and pregabalin relieve severe itching in most patients with chronic kidney disease , investigators reported at the 50th Congress of the European Renal Association-European Dialysis and Transplant Association.

Our results bolster earlier data in support of a role for gabapentin and pregabalin in the treatment of a problem which is much more than intermittent irritations relieved by a satisfying scratch, said Hugh C. Rayner, MD, consultant nephrologist at Heart of England National Health Service Foundation Trust in Birmingham, U.K.

His team used gabapentin and pregabalin in 71 consecutive patients with CKD stage 4 or 5, some of whom were on hemodialysis or peritoneal dialysis .

Uremic pruritus, also referred to as CKD-associated pruritus, is a common symptom in patients with end-stage renal disease, Dr. Rayner observed. In severe cases, the symptoms may disrupt sleep and decrease quality of life, which is associated with depression and increased mortality.

Gabapentin and a related drug, pregabalin, both of which are primarily used for the treatment of epilepsy and neuropathic pain, have been shown to control itching in small randomized controlled studies involving HD patients.

Overall, gabapentin or pregabalin relieved itching in 60 patients, or 85%. Patients had a median itch severity score of 8 before treatment and 1 after treatment.

What Are The Symptoms Of Pruritus

The main symptom of pruritus is an itch. An itch is a feeling or uncomfortable sensation that causes you to scratch your skin to get relief. Scratching the itch doesnt always provide relief.

What did we find?One type of drug , an analogue to a common neurotransmitter appear to reduce itch in patients with CKD. Ondansetron, an anti-nausea drug, was another well studied treatment and appears have no significant association with itch reduction. Kappa-opioid drugs appear to slightly reduce itch. There is too little information on the remaining treatments for any thorough assessment of their efficacy in relieving itch or whether there is any anti-itch effect at all.

The three drugs mentioned above are well studied with higher quality evidence. The other treatments studied are of lower to moderate quality.

The studies seldom document a comprehensive list of adverse or side effects incurred during treatment. However, none of the adverse effects documented were severe. Further meaningful assessment on harm cannot be made.

Drugs that work like neurotransmitters reduce itch in patients with CKD. Other intervention either do not work, do not work as well, or need further study to make a conclusion.
