Scar Improvement Enhanced With Intralesional Triamcinolone Acetonide Injection During Static Stage

Injection of intralesional triamcinolone acetonide is more effective during the static stage of pathologic scarring than during the early stage, according to study results published in the Journal of Cosmetic Dermatology.1

The investigators sought to elucidate whether the effect of intralesional triamcinolone acetonide injection during the early stage of scarring differs from the effect during the static stage.

A total of 108 individuals with pathologic scars were enrolled in the study. The participants were divided into 2 groups based on the time of their first treatment following injury: the early-stage and the static-stage group . Patients from both groups were then treated with intralesional triamcinolone acetonide injection.

The Vancouver Scar Scale was used to evaluate the scars,2 a durometer was used to measure scar hardness, and the Visual Analog Scale was used to evaluate patients subjective feelings . Treatment effectiveness was divided into 4 groups: ineffective, < 25% mildly effective, 25% to 50% moderately effective, 50% to 75% and very effective, 75%).

Based on effectiveness percentage, most of the scars were improved with respect to color, thickness, softness, and vascular distribution following treatment, with an effective rate of 87.2% . In the moderately effective group, the overall efficacy in the static-stage group was statistically significantly superior to that in the early-stage group .

How Is A Hydroquinone Tretinoin Cream Used

It can take at least 4 weeks and even up to 8-12 weeks tonotice results from using a hydroquinone tretinoin cream. Application is oftendone once daily at bedtime. A broad-spectrum sunscreen is recommended for dailyuse as well. Many patients will experience mild side effects like redness anditching. While more severe side effects are possible, they are not common. Treatmentwith these creams continues for a length of time determined by the patientsdermatologist. Regular checkups help make sure the treatment is working andthat there are no adverse effects.

In studies of hydroquinone and tretinoin, patientsatisfaction rates have been high. A study in the Journal of Drugs inDermatology with hydroquinone 4% and tretinoin showed that at week 24, 87.9% ofpatients were satisfied or very satisfied with their treatment. For higherstrengths of hydroquinone, continuing treatment for two years may not bepossible for all patients. Some doctors will use treatment courses lasting 3-6months, with time in between treatment courses to allow the skin to adjust. Othersmay prescribe the cream to be used only two or three times a week.

Triple ingredient lightening creams can take time to work, so it is important for patients not to get discouraged if they do not see the results they want after a month of treatment. The skin needs time to replace the hyperpigmented skin cells with new skin cells for the results to be visible.

Over The Counter Creams

Does Triamcinolone Acetonide Cream Lighten Skin

The active ingredients to look for in over the counter cream to lessen the appearance of dark spots are vitamin C, niacinamide, alpha hydroxy acids and licorice root.

Vitamin C creams can be used as a topical antioxidant. These formulations help to block free radicals from causing oxidative damage to the skin. Vitamin C inhibits enzymatic processes that produce melanin in the skin and as such it can help to reduce dark coloration of the skin, Sumayah Jamal, M.D,. Of Schweiger Dermatology Group reported to SELF.

Another vitamin you may see in over the counter creams to help with dark spots is niacinamide. Niacinamide is the active form of niacin or vitamin B3. As a water soluble vitamin, it wont dissolve in oil and is perfect for anyone who cant tolerate oils and oily solvents. Since niacinamide readily penetrates the skin it can be used topically. Its benefits are similar to vitamin C in that it prevents oxidative stress and has been proven to be an excellent treatment for pigmentation, age spots, and overactive melanin.

How Should This Medicine Be Used

Triamcinolone comes in ointment, cream, lotion, and aerosol in various strengths for use on the skin and as a paste for use in the mouth. It usually is applied two to four times a day. For mouth sores, it is applied at bedtime and, if necessary, two or three times daily, preferably after meals. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Use triamcinolone exactly as directed. Do not use more or less of it or use it more often than prescribed by your doctor. Do not apply it to other areas of your body or use it to treat other skin conditions unless directed to do so by your doctor.

To use triamcinolone topical, apply the ointment, cream, or lotion sparingly in a thin film and rub it in gently.

To use the lotion or aerosol on your scalp, part your hair, apply a small amount of the medicine on the affected area, and rub it in gently. Protect the area from washing and rubbing until the lotion or spray dries. You may wash your hair as usual but not right after applying the medicine.

To apply an aerosol, shake well and spray on the affected area holding the container about 3 to 6 inches away. Spray for about 2 seconds to cover an area the size of your hand. Take care not to inhale the vapors. If you are spraying near your face, cover your eyes.

If you are using triamcinolone topical on your face, keep it out of your eyes.

Getting The Most From Your Treatment

    • If you are using a moisturiser along with this preparation, apply the moisturiser first. Then wait 10-15 minutes before applying Aureocort® ointment. This allows time for the moisturiser to be absorbed before the topical corticosteroid is applied. Your skin should be moist but not slippery when you apply Aureocort®.
    • Do not use Aureocort® on your face unless a skin specialist doctor has said you should. If you have been told to use it on your face, be careful not to get any preparation near your eyes and do not use it for longer periods of time than you have been advised.
    • Unless advised to do so by your doctor, do not apply a bandage or dressing to the area being treated, as this will increase absorption of the preparation and increase the risk of side-effects.
    • Continue to use Aureocort® until the flare-up has gone and then stop it. A course of treatment for seven days is usually sufficient. If your symptoms have not improved after this time , speak again with your doctor for further advice. Topical corticosteroids like triamcinolone should not be used for long periods of time or on large areas of the body.
    • Be careful not to let the preparation come into contact with your clothing, as it may stain.

    Alternatives For Skin Lightening

One of the reasons that hydroquinone tretinoin creams arepopular for skin lightening is that they are cost effective. It has been inshown in some studies that hydroquinone combined with laser therapy can getfaster results than when it is used alone. Laser therapy is expensive, though,as are other in-office treatments for hyperpigmentation. Some patients may wantto use everything that their dermatologist has available including lasers, chemical peels, creams, and other treatments. However many patients will accepta slightly longer treatment time with a lower cost and no downtime.
