What Is Metastatic Melanoma
Melanoma is a cancer that begins in the melanocytes . Metastatic melanoma is considered to be a late form of stage IV of melanoma cancer and occurs when cancerous melanoma cells in the epidermis metastasize and progress to other organs of the body that are located far from the original site to internal organs, most often the lung, followed in descending order of frequency by the liver, brain, bone and gastrointestinal tract 1). The two main factors in determining how advanced the melanoma is into Stage IV are the site of the distant metastases and whether or not the serum lactate dehydrogenase level is elevated. LDH , an enzyme found in your blood and almost every other cell of your body, turns sugar into energy, and the more you have in your blood or other body fluid, the more damage has been done to your bodys tissues.
It is crucial to diagnose melanoma in its early stages before it metastasizes, as once it has spread, it is difficult to locate its origin and so treatment and patients survival rate tends to be hindered 2).
An estimated 178,560 cases of melanoma will be diagnosed in the U.S. in 2018 3). Of those, 87,290 cases will be in situ , confined to the epidermis , and 91,270 cases will be invasive, penetrating the epidermis into the skins second layer 4).
Melanomas can develop anywhere on the skin, but they are more likely to start on the trunk in men and on the legs in women. The neck and face are other common sites.

Factors Associated With Overall Survival
At the time of data lock, 182 patients were deceased and 108 were still alive, while the other 15 resulted lost in follow-up. The median OS was 10.7 months . Table 5 describes data from both univariate and multivariate analyses of the factors potentially correlated with prognosis. The univariate analysis revealed a worse prognosis in males and in patients with melanoma of the trunk as well as a number of five or more BMs , high LDH levels at metastatic BD diagnosis , and evidence of three or more metastatic sites , or brain metastasis . In addition, patients who received new agents showed OS improvement with respect to those treated with CHT . In addition, neither detrimental impact on OS was determined by the occurrence of SREs, nor in the case of multiple SREs . The multivariate analysis confirmed as positive strong independent prognostic factors less than five skeletal metastasis , limited LDH values , less than three metastatic sites , and treatments with targeted agents and/or ICIs .

Symptoms Of Metastatic Melanomas
Melanoma usually is found in early stages, before its become metastatic. If you notice any abnormal moles or discolorations on your skin, dont hesitate to reach out to your doctor. This is especially important for those with many risk factors. Melanoma is more treatable at early stages, so early identification may prevent metastatic melanoma from developing.
Though a primary tumor is typically found, its possible that metastatic melanoma is detected elsewhere in the body and causes symptoms without any signs of a primary tumor.
Metastatic melanoma symptoms and signs may include:
- Fatigue
What Are The Symptoms Of Melanoma That Has Spread
Melanoma that has spread from the skin to other areas of the body is known as metastatic melanoma. However, since melanoma often first presents itself as an abnormal mole, many people with this malignancy can receive a diagnosis before the cancer has spread. This mole may be asymmetrical, have an uneven border, have an inconsistent color, be large or change over time. A melanoma may also appear as a sore or itchy bump, a tender nodule or a patch of skin that is scaly or bleeding. In some cases, early signs of melanoma are not present. For example, if the cancer starts in a mucous membrane rather than on the skin, a mole may not be present.

Canine Malignant Melanoma Etiology
In humans, malignant melanoma is the most aggressive, therapy-resistant, and deadly form of skin cancer . The major risk factors for human melanoma include family history, skin and mucosal pigmentation characteristics, sun exposure, particularly to UVB light, and skin reactions to sun exposure. Another type of melanoma, oral malignant melanoma, is a rare aggressive neoplasm usually seen in middle-aged adults.
Several etiological factors are supposed to be involved in canine malignant melanomas, including consanguinity, trauma, chemical exposure, hormones, and genetic susceptibility . However, there is no consensus regarding the etiology of malignant melanomas in dogs . Sunlight may be involved in the development of this disease in the sun-exposed skin areas of the body, such as the face and pinnae however, sunlight probably is not involved in mucosal melanomas, like the ones found in the canine buccal cavity. Other factors, like the presence of pigmented cells, trauma, chemical agents, or even the buccal microbiota, and inflammation may be associated with the etiology of these tumors .
Outlook Once Cancer Has Spread To The Bones
The research on cancer metastasis is rapidly growing. As researchers better understand the mechanisms of bone metastasis, new drugs and other treatments are being developed. These target particular processes in cells involved in how the cancer cells invade and grow in bones.
The use of nanoparticles to deliver drugs is very encouraging. These tiny particles are able to deliver drugs to the bone with minimal toxicity to the person with cancer.
Rapidly treating bone metastasis can lead to a

What Are The Treatments For Metastatic Melanoma
Ideally, melanoma is diagnosed and treated surgically while it is still small and thin and before it has had the chance to metastasize. The prognosis and survivability of metastatic melanoma remain poor compared to other types of cancer. Metastatic melanoma is less responsive to radiation therapy and traditional forms of chemotherapy than other forms of cancer.
Immunotherapy in which the body’s own immune system is used to fight the tumor has been a focus of research for decades and immune system “stimulants” such as interferon-alpha and interleukin-2 have also been tried for many years.
A variety of newer medications target different points in the biologic pathways of melanoma growth and spread. The following are drugs currently in use, or being actively investigated. More should be available shortly.
- Inhibition of kinase enzymes needed in cell reproduction like MEK: cobimetinib , trametinib
- Target cell growth signals from abnormal BRAF genes: dabrafenib , vemurafenib , nivolumab
- Improve immune response to the tumor: pembrolizumab , ipilimumab
As monotherapy , these drugs have not been shown to dramatically improve survival even if they did improve disease-free time. The hope is that combinations of medications targeting more than one part of the melanoma growth and metastasis pathway will provide more encouraging results.
Survival Rates Of Bone Metastases
Survival rates for people with bone metastases vary greatly by cancer type and stage. Your general health condition and the type of treatment you received for the primary cancer are additional factors.
Discuss your particular situation with your doctor. Remember that survival rates are averages gathered from large numbers of people. Also, survival data may reflect statistics from a period before the most recent treatment advances.
A large-scale 2017 study of the 10 most common cancers with bone metastasis found:
- Lung cancer had the lowest 1-year survival rate after bone metastasis .
- Breast cancer had the highest 1-year survival rate after bone metastasis .
- Having metastases in bone and also in other sites was found to decrease the survival rate.
Here are some typical figures from a 2018 study of common cancers and bone metastasis:
Type of cancer |
Youre likely to have a combination of therapies that may include:
- radiation to slow metastasis growth and reduce pain
- chemotherapy to kill cancer cells and reduce tumor size
- hormone therapy to reduce the hormones known to be involved with breast and prostate cancer
- painkillers and steroids for pain relief
- drugs that specifically target bones
- surgery if necessary to stabilize your bone, fix a break, and help with pain
- physical therapy to strengthen your muscles and help you with mobility
- extreme heat or cold that targets cancer cells and may relieve pain

Complications Caused By Treatment
The treatments for metastatic melanoma can cause nausea, pain, vomiting, and fatigue.
Removal of your lymph nodes can disrupt the lymphatic system. This can lead to fluid buildup and swelling in your limbs, called lymphedema.
Some people experience confusion or mental cloudiness during chemotherapy treatment. This is temporary. Others may experience peripheral neuropathy or damage to the nerves from the chemotherapy. This can be permanent.
Red Flag #: Bone Pain Or Fractures
The bones are considered a late-stage site of melanoma metastasestypically, it doesnt spread to the bones until its already spread to another area of the body first. Melanoma can cause pain in the bones where its spread, and some peoplethose with very little body fat covering their bonesmay be able to feel a lump or mass. Metastatic melanoma can also weaken the bones, making them fracture or break very easily. This is most common in the arms, legs, and spine. If you feel any sharp, sudden, or new pains that wont go away, talk to your doctor.

Stop Tumors In Their Tracks
Every melanoma has the potential to become deadly, but the difference between an in situ melanoma and one that has begun to metastasize cannot be overstated. There is a drastic change in the survival rate for the various stages of tumors, highlighting the importance of detecting and treating melanomas before they have a chance to progress. Its impossible to predict exactly how fast a melanoma will move from stage to stage, so you should be taking action as soon as possible.
To be sure youre spotting any potential skin cancers early, The Skin Cancer Foundation recommends monthly skin checks, and scheduling an annual total-body skin-exam with a dermatologist. These skin exams can help you take note of any new or changing lesions that have the potential to be cancerous, and have them biopsied and taken care of before they can escalate.
Trust your instincts and dont take no for an answer, Leland says. Insist that a doctor biopsy anything you believe is suspicious.
How Does The Doctor Know I Have Skin Cancer
Basal and squamous skin cancer may look like:
- Flat, firm, pale or yellow areas that look a lot like a scar
- Raised reddish patches that might itch
- Rough or scaly red patches, which might crust or bleed
- Small, pink or red, shiny, pearly bumps, which might have blue, brown, or black areas
- Pink growths or lumps with raised edges and a lower center
- Open sores that dont heal, or that heal and then come back
- Wart-like growths

Intravenous contrast was administered in patients with normal renal function.
How Fast Does Dog Spleen Cancer Spread
Spleen cancer is also a cancer that quickly spreads. In this case, the cancer has a tendency to spread quite fast to the dogs liver . When there is spread to the liver, removal of the spleen might not be worthwhile, explains veterinarian Dr. Lee.
With spleen cancer, survival rates once the tumor has spread are not very promising. A dog is lucky to get 3 to 6 months, points out veterinarian Dr. Andrea Robert. In some cases, it can even be less, even just one single month. The secondary cancers in the liver can start to bleed within a very short time. With this type of cancer, bleeding is a main issue.
Extensive bleeding from the liver or spleen can cause shock to rapidly set in. If surgery is not pursued, dogs need to be prevented from bumping the abdomen as it can happen when jumping off the couch or tripping down the stairs.
A secondary cancer to the liver can be ruled out or confirmed through an x-ray and ultrasound done before surgery.
If it is one of the more nasty cancerous tumors it usually will spread to other organs very quickly If no spread is obviousthen Id recommend surgery. There is always a chance that the ultrasound will be normal and your vet will still find spread in surgery ~Dr. Dan, veterinarian.
How Fast Does Oral Cancer Spread in Dogs?
Dogs with stage 2 oral melanoma measuring between 2 and 4 cm that has not spread, tend to have a median survival time of 5 to 6 months following surgical removal of the mass.

Melanoma Metastasis In The Bone
When melanoma has spread beyond lymph nodes to distant organs, it is called Stage IV disease. According to the American Cancer Society, 5-year survival rates are 15-20%, and 10-year survival is 15-20%. It is estimated that melanoma has 23-49% probability of metastasizing to skeletal locations. This is called melanoma bone metastasis or bone mets.
The first symptom of bone mets is often bone pain. Other symptoms include bone fractures, and elevated blood levels of calcium as the cancerous tumors in the bone break the bone down which releases calcium into the bloodstream. Any bone pain symptom should be immediately evaluated by a doctor.
Red Flag #: Abdominal Pain And Tenderness
Early on, there may be no noticeable symptoms that melanoma has spread to the liver. When symptoms do show up, they commonly include an enlarged, hard, or tender liver and pain in the upper right area of your abdomen, just below your ribs. Other signs cancer has spread to the liver are similar to symptoms of liver disease: fluid buildup in the belly and yellowing of the skin and eyes .

Biology Of Dog Cancer
In order to better understand how fast cancer spreads in dogs, it helps to comprehend how cells normally communicate. After grasping this concept, one can get a better understanding on the complexity involving the spread of cancer in dogs.
In a normal, healthy dog, cells are programmed to undergo several events. The cells start out as immature cells, then they mature, multiply and finally reach their main purpose. All cells have a specific purpose during their lifespan.
Cells that are damaged are also repaired and those that are no longer needed are replaced. This natural process is known as apoptosis.
Cancer cells, on the other hand, do not follow the cycle that nature has programmed. While normal cells stop reproducing when there are enough cells, cancer cells do not. For example, lets imagine a cut on a dogs paw. Cells are produced to repair the cut in the skin, but the production will come to a halt once there are enough cells to fill the gap in the skin. Cancer cells instead will keep growing and may eventually form a cluster of cancer cells . Cancer cells also do not go through the process of being repaired or dying when they are damaged or old .
Did you know? Disease and cancer is suspected to occur when the process of apoptosis malfunctions and cells do not die as they should. Some supplements for dogs with cancer like Dr. Dresslers Apocaps can help induce apoptosis in cancer cells.
Common Places For Melanoma To Spread
Melanoma can spread from the original site on your skin and form a tumor in any organ or body tissue, but its most likely to metastasize to the lymph nodes, liver, brain, lungs, and less commonly, the bones. Melanoma really likes the brain and the liver, says Lisa Zaba, M.D., dermatologic oncologist at Stanford Medical Center in San Jose, CA. If you notice any of the following red flags, it might mean your melanoma has spread and warrants a call to your doctor right away.

What Is Brain Metastases
Brain metastases, a specific form of Stage IV melanoma, are one of the most common and difficult-to-treat complications of melanoma. Brain metastases differ from all other metastases in terms of risk factors, diagnosis, and treatment.
Until recently, melanoma brain metastases carried a poor prognosis, with a median overall survival of about four to five months, but improvements in radiation and systemic therapies are offering promise for this challenging complication, and some patients are curable. Historically, people with a single brain metastasis who undergo effective treatment have a better chance for long-term survival than do people with multiple metastatic tumors.

Different Kinds Of Skin Cancer
There are many types of skin cancer. Some are very rare. Your doctor can tell you more about the type you have.
The two most common kinds of skin cancers are:
- Basal cell cancer, which starts in the lowest layer of the skin
- Squamous cell cancer, which starts in the top layer of the skin
Another kind of skin cancer is called melanoma. These cancers start from the color-making cells of the skin . You can read about melanoma in If You Have Melanoma Skin Cancer.
